Reminder of CRI's Confidentiality Clause signed by each provider during licensing:
Confidentiality Statement

I understand that by being a foster/adoptive parent, all information given to me regarding a child in my care is confidential. I agree not to share my child's personal history or information with schools, relative family members, or anyone other than the child's DCF social worker, therapist, or CRI staff. I also agree not to discuss any issues with the child while in the community. If I have any questions or concerns regarding the child, I will discuss it only with the professionals involved with the child's care as listed above.

I further understand that becoming a foster/adoptive parent involves sharing personal information about myself and my family, including family, medical, and mental health history. Information that I have provided will be maintained in a confidential manner by CRI staff. CRI staff may speak to one another about such information for the purpose of improving care to clients while involved in CRI programs, but information will not be discussed outside of CRI. Files will also
be made available to state agencies for the purpose of quality assurance, accreditation, and to comply with agency licensing requirements. I understand CRI staff may share information, including my home study, with DCF for purposes of matching a child to be placed in my home
and ongoing care of the child. I understand that CRI will notify me and request my written consent before sharing information with anyone or for any purposes not described above.

I, have read and fully understand the above confidentiality statement. My signature indicates my agreement that will maintain the confidentiality of any child in my care, and my approval for confidential information I provide about myself to be maintained by CRI in the manner described.

This note is just serving as a reminder to our families to please be careful not to discuss any personal information about the child in your care within this forum. We want to respect our clients confidentiality. Thank You!
